Siena Woods understands that choosing a skilled nursing facility often comes during a time of need. So, we strive to make the admissions process as smooth and stress-free as possible – for both patients and families. From your very first day, our compassionate team is here to support you with high-quality care and open communication every step of the way.
For your convenience, we offer admissions 24 hours a day, seven days a week – because we know that care can’t always wait for regular business hours.
What to Expect Upon Admission
When you arrive at Siena Woods, our nursing staff will greet you and begin your initial assessment, including checking your vital signs and reviewing your care needs. We’ll also complete an inventory of clothing and personal items to help ensure everything stays organized during your stay.
Within the first 72 hours, you’ll meet with key members of our Administrative Team. These brief introductions will provide helpful information and introduce you to important aspects of your care plan, including:
- Anticipated length of stay
- Medications and treatment plans
- Insurance and payor information
- Therapy and rehabilitation goals
- Discharge planning
- Advanced directives
- Care plan meetings and reviews
We aim to provide clarity and reassurance from day one, so you always know what to expect.
What to Bring (and What to Leave at Home)
We want your stay at Siena Woods to feel as comfortable and familiar as possible. Bringing a few personal items can help you feel more at ease, but safety and care guidelines mean there are also a few things best left at home.
Suggested Items to Bring
- Washable and labeled clothing (7- to 10-day supply) and supportive walking shoes
- Personal toiletries
- Eyeglasses, dentures, and hearing aids
- Plastic-framed family photos
- Personal furniture (as space allows)
- Lightweight décor like blankets, throw pillows, or artwork
- Disposable incontinence products
- Portable music devices (e.g., MP3 players, small audio players, CDs)
Important Documents to Bring:
- Active medical insurance card and Medicare/Medicaid cards
- Part D (pharmacy) prescription coverage card
- Social Security card
- Driver’s license or other government-issued photo ID
- Advance Directives (e.g., DNR paperwork)
- Power of Attorney documents (medical, financial, or durable)
- Guardianship paperwork, if applicable

- Breakable items (glass, ceramics, etc.)
- Electric blankets or heating pads
- Bedside medications
- Powder-based toiletries
- Perishable food or live plants
- Power strips or extension cords

Your Medical Team
During your stay, your care will be overseen by a dedicated team of RNs, LPNs, social workers, and licensed therapists, all guided by our attending physician and nurse practitioner. This skilled team will:
- Complete a comprehensive medical assessment
- Develop and monitor your individualized care plan
- Order and review diagnostic tests as needed
- Provide routine and emergency medical oversight
Your first physician visit will typically take place within 72 hours of arrival, with follow-up visits scheduled every 30 days or more frequently if required. Our medical team is also on call 24/7 to ensure your needs are always addressed promptly.
Medication Management
After clarifying your discharge orders from the hospital, your prescriptions will be sent to our partner pharmacy. Medications are typically delivered to the facility three times per day, so most medications are available the same day you arrive. Your nurse supervisor is always available to discuss any questions about your medication plan.
Therapy Services
Shortly after admission, you’ll be evaluated by our therapy team to assess your functional status and establish goals. Based on your individual care plan and physician’s orders, you may receive therapy services five to six days a week, with one to three sessions per day. Each session ranges from 30 to 75 minutes. A customized therapy schedule will be provided to you daily and is also available at the nurses’ station.
Discharge Planning
Prior to your discharge, our Social Services Director will work with you, your family, and your care team to arrange post-discharge services to ensure a safe, well-coordinated transition back home or to the next level of care. These services may include:
- In-home care services
- Outpatient therapy or nursing
- Medical equipment and supply coordination
- Follow-up care planning and referrals
