Emotional and mental well-being play a critical role in overall health, especially for individuals living in long-term care settings. Exceptional Care Services provides a structured Behavioral Health Model of Care that integrates mental health support directly into everyday medical care.
Our team includes Psychiatric Mental Health Nurse Practitioners (PMHNPs) and Licensed Clinical Social Workers (LCSWs) who work collaboratively with residents, families, and facility staff to create custom care plans that promote stability, comfort, and engagement.
Behavioral Health Support Includes:
- Diagnosis and management of depression, anxiety, bipolar disorder, schizophrenia, chronic pain, and dementia-related conditions
- Individualized behavioral health care planning
- Ongoing assessment of psychosocial, emotional, and mental health needs
- Coordination with facility teams and community resources
