What We Do
Care Tailored to You
Primary Care ServicesOur dedicated primary care nurse practitioners are on-site and available to residents throughout the week, providing timely assessments, preventive care, and treatment for both acute and chronic conditions. With a comprehensive treat-in-place model, many services typically requiring a hospital visit can be delivered right at the bedside. Residents can stay comfortable in familiar surroundings while receiving high-quality medical care.
Behavioral Health Services
Exceptional Care Services offers an integrated behavioral health model that supports residents’ emotional, psychological, and cognitive well-being. Our Psychiatric Mental Health Nurse Practitioners (PMHNPs) and Licensed Clinical Social Workers (LCSWs) work collaboratively with facility teams to deliver individualized care plans that address conditions such as depression, anxiety, dementia-related behaviors, and more.
Chronic Care Management
For residents living with multiple chronic conditions, our Chronic Care Management (CCM) program provides ongoing coordination, monitoring, and support. By combining medical oversight with behavioral health integration, we help residents better manage their health, reduce complications, and focus on living each day with greater stability and confidence.
