Our Primary Care Services
Our dedicated primary care nurse practitioners focus on preventive wellness services that are personalized with the goal of keeping you or your loved one healthy by avoiding illness.
We recognize that illness is a part of life sometimes and we are well trained to provide many of the same services a hospital can provide right at the bedside. Our comprehensive treat-in-place model reduces unnecessary hospital transfers so that our patients can stay focused on what matters most in the comfort of their own surroundings.
Services Include:
- Acute symptom management
- Chronic disease management
- IV antibiotic and fluid therapy
- Joint injections
- On-site laboratory testing
- Minor surgical procedures
- Pain management
- Palliative and end-of-life comfort care
- Regulatory visits (alternating with physician visits)
Benefits of Having a Full-Time Primary Care NP On-Site:
- Five-day-a-week on-site availability for timely assessments and treatments
- Reduction in ER visits and re-hospitalizations through proactive care
- Active participation in clinical team meetings and Quality Assurance and Performance Improvement (QAPI) programs
- Review of all new admissions, readmissions, ER visits, and outpatient consults
- Comprehensive assessment and treatment of chronic and acute conditions, including medication management
- No cost to the facility – NP services are billed directly to payer sources like Medicare or Medicaid
Our Behavioral Health Services
Exceptional Care Services also provides a structured Behavioral Health Model of Care that supports residents’ emotional and psychological well-being. Our team includes Psychiatric Mental Health Nurse Practitioners (PMHNPs) and Licensed Clinical Social Workers (LCSWs) who work closely with facility teams to deliver personalized behavioral health services.
Behavioral Health Support Includes:
- Diagnosis and management of depression, anxiety, bipolar disorder, schizophrenia, chronic pain, and dementia-related conditions
- Creation and implementation of personalized care plans
- Ongoing evaluation of psychosocial, emotional, and mental health needs
- Connection to services that support well-being and improve quality of life

The Exceptional Care Services Team

Jeff Beaty, PhD, LMSW, MSHA
Senior Vice President Behavioral Health
Jeff joined Exceptional Care Services in July 2023 as the Senior Vice President of Behavioral Health. He is responsible for establishing the behavioral health division, monitoring progress with the model and providing support and leadership.
Jeff critically analyzes clients with major neurocognitive disorders, dementia, and mental health disorders to improve quality of care. He provides active functional support and guidance in inter-operation of medical policy for associates to meet the healthcare and behavioral health needs of the clients including those within a palliative care situation.

Denise Kirkham
Chief Practice Officer
Denise Kirkham joined Exceptional Care Services in February 2023 as the Chief Practice Officer. Denise provides leadership for the Exceptional Care Services Advanced Practice Providers group.
Denise has over 40 years of healthcare experience providing strategic and operational health care solutions. Her focus is on quality and cost-effective measures to improve healthcare outcomes, and provider and patient satisfaction.

Ray Avery, MSN, APRN, FNP-C
Director of Medical Services
Ray has worked in nursing centers, assisted living facilities, and rehabilitation centers for over 14 years. He started his healthcare journey as a nursing assistant. As he continued his education, he was afforded new opportunities and worked as a registered nurse, a facility nursing manager, and has served in multiple leadership roles. Ray graduated from Walden University with a master’s degree in science, which allowed him to start practicing as a nurse practitioner. In his role as the Director of Medical Services, he leads our remarkable team of primary care nurse practitioners with a focus on improving clinical outcomes.
